All registration forms must be signed and dated. If you register by e-mail or telephone, we must set up an appointment to have you sign your form.

All registrants must be 18 years of age or over.

Please PRINT clearly and use a separate form for each registration.

TO ENSURE COMPLETE CONFIDENTIALITY, If you are paying using Visa or Mastercard PLEASE check the appropriate box on the form and we will contact you for the CREDIT CARD DETAILS. IF PAYMENT IS TO BE MADE BY CHEQUE OR MONEY ORDER, PLEASE ENCLOSE WITH THE REGISTRATION FORM WHEN MAILING.

2. By Phone:

Payment by Visa or Mastercard IS PREFERRED. We will complete the registration form over the phone, so please allow enough time to get the details required. PLEASE HAVE YOUR VISA OR MASTERCARD AT HAND WHEN CALLING.

3. In Person:

If payment is being made in person, GIVE US A CALL TO MAKE AN APPOINTMENT before attending at 1071 King Street West (at Sudbury), Suite 107, Toronto. WE CAN BE REACHED AT register@bluechipconnection.com or (416) 260.2282 to set up a time. Appointments can be accommodated outside of office hours when necessary.
Any payment using a Debit card or Cash must be made in person. DO NOT SEND CASH THROUGH THE MAIL.

SEND YOUR COMPLETED REGISTRATION FORM TO (416) 345-9044 and we will contact you to obtain YOUR CREDIT CARD INFORMATION (Visa or MasterCard.)

Please Note: A $35 service charge will apply to any dishonored cheques. Any subsequent payment will be on a CASH ONLY basis.

 

a) A cancellation fee of $50.00 per registration will be charged to anyone withdrawing from the program once REGISTRATION HAS BEEN ACCEPTED.

b) Once the pre-interview has taken place, a cancellation fee of $200 will be charged FOR WITHDRAWING FROM THE PROGRAM.

c) Anyone withdrawing from the program IN THE TWO WEEKS PRIOR TO CLASSES STARTING WILL FORFEIT THE REGISTRATION FEE IN FULL.

d) THERE ARE NO REFUNDS AFTER THE CLASSES HAVE COMMENCED.

e) Spaces are reserved on a first come first serve basis. TO RESERVE A SPACE, YOUR REGISTRATION FORM MUST BE COMPLETED IN FULL AND PAYMENT MUST HAVE BEEN MADE. You will be notified of the class date. If you wish to specify a date convenient to you, contact us so we can try to accommodate you.

f) All fees are non-transferable under any conditions.

g) Days missed cannot be refunded.

h) Parents, guardians and students agree to abide by all rules, regulations, financial policies and procedures and standards of conduct.
